PATHS (Person-centered Alternatives To Help Students) offers transition opportunities for students as they prepare to leave school. "Transition" is defined as a coordinated set of activities which promote movement from school to post-school services for the student with a disability. Working with the student's school and family, our staff of professionals will develop a plan for services before the student leaves school.
Person-Centered Planning is a process in which a student's talents and interests are considered first. This allows a better determination as to which services will help the student move closer to their own goals.
Service Options
Day Habilitation is designed to assist an individual in being a productive member of the community. Several small sites provide a base to support adults of all ages, abilities and interests.
Lifestyles, a component of Day Habilitation, offers classes that promote individuality, independence, inclusion and productivity. Classes include performing arts, computer skills, cooking, gardening and fine arts.
Vocational Services may begin while a student is still in school and offer work experience in a variety of situations. Services include Work Try-Out, Community-Based Assessment, Career Counseling, On-Site Individualized Training and Job Placement.
