

Medicaid Service Coordination is available to people who live independently, with their family, or in a residential setting. Service Coordinators work side-by-side with individuals and families to help them learn about the wide variety of services and supports available. Service Coordinators help people access, maximize and manage financial and medical benefits. By providing ongoing information on education, housing, employment, social and recreational opportunities, Service Coordinators help people make the best choices.
Intensive Service Coordination is a special type of service coordination available for individuals with a learning disability and a mental illness. This program, currently available only in Seneca County, is designed to assist in meeting the unique needs of individuals by coordinating community-based services from both the Office of Mental Retardation & Developmental Disabilities and the Office of Mental Health.
Early Intervention Service Coordination is ongoing service coordination for children (ages 3 and younger) who qualify through the Seneca County Department of Health Early Intervention Program. Early Intervention Service Coordinators provide support and advocacy to families of very young children experiencing developmental delays. Staff work closely with family members and clinicians to identify and serve the needs of the child.
